Location
The 
Flame Leviathan is the first boss of Ulduar.
Strategy
After moving through the gauntlet area filled with Iron Dwarves leading up to the Formation Grounds, players just outside the Formation Grounds will find stationary vehicle repair disks (which look like large versions of the teleporters within the instance). Driving over these disks will restore your vehicle to maximum health.
To gain this achievement, simply avoid driving over these repair disks with any vehicle in your group, start the encounter with Flame Leviathan and defeat it.
