This item drops off the Silithid Royalty in the Temple of Ahn'Qiraj in Silithus.

Notes

It is shared loot; you don't need to kill them in any specific order.
Even after the release of Naxxramas, this item was the best tanking helmet for druids until the release of The Burning Crusade.
