Inv misc leatherscrap 08 10x [Thick Leather] Inv potion 12 6x [Flask of Big Mojo]
Inv fabric silk 02 2x [Heavy Silken Thread]

The pattern drops off creatures in Zul'Farrak, Maraudon, Blackrock Depths, and The Temple of Atal'Hakkar.

Notes

Although not an actual set, there are several other Big Voodoo items: Inv banner 01 [Big Voodoo Mask], Inv misc cape 02 [Big Voodoo Cloak], and Inv chest cloth 25 [Big Voodoo Robe].

Patch changes

Cataclysm Patch 4.1.0 (2011-04-26): No longer requires Inv potion 12 [Flask of Big Mojo].
